Common Forms
Here are links to frequently used online forms in the Isenberg School.
If you are not sure which form is the appropriate one for your request please read the descriptions below to help you chose the correct one.
Use the Course Override Request Form to request a class that you need but cannot register for on SPIRE. Be sure to explain why you need the class and why SPIRE is not allowing you to register.
Use the Credit Overload Request Form to request an increase in your credit limit. In the fall and spring semesters, if you are full time and want to take more than 18 credits or part-time and want to take more than 11 credits, you must request a credit overload. Over 7 credits per summer session requires an overload. In a wintersession, over 4 credits constitutes an overload-NOTE Wintersession is extremely intensive, you should expect to have to put in at least 40 hours of course work per week, per course!
Use the Late Course Withdrawal Petition during the add/drop period, you should use Spire to drop any course that you don't want. Following add/drop, you can drop any course with a "W" by using the University's Course Change Request form. You should get your instructor's signature to let him or her know you are dropping the course, then bring the signed form to the Registrar's office or to SOM 206, and we will send it to the registrar for you. After the "W" period (mid-semester), you must submit this petition in order to drop a course. Furthermore, there must be extenuating circumstances to justify your request, e.g. illness, family emergency, etc. Use the text box at the end of the form to explain.
Use the Pass/Fail Petition Form after the add/drop period to request that a course be changed from graded to pass/fail. Remember, you cannot fulfill specific requirements with a pass/fail course. Pass/fail courses must be electives.
