If the stock certificate is in the name of the donor:
The donor must fill out a stock power transferring ownership of the certificate to the University of Massachusetts Amherst. Donor signature on stock power must be guaranteed (not the same as notarized) and/or medallion stamped. Signature guarantee should be made by a member or member organization of the New York Stock Exchange. Most banks can provide this service, but call ahead to make sure.
Certificate and stock power should be mailed in separate envelopes.
